This form does not activate partner access. It confirms identity, consent, and the requested role for Governor review.

Role Accepted * You may select a lower role than nominated. The Governor approves final access.

Partner involvement is subordinate to Greywell authority. The Governor remains final authority, the House Officer remains the administering interface, and partner access may be denied, reduced, suspended, or revoked. Welfare reports, medical details, payment information, internal notes, and private Governor records are not disclosed through partner access.
